Auspice body report confirmation

This form is to confirm that the auspice body are aware of and agree with the reporting provided by the applicant, for the Department of Communication and the Arts Grant that the auspice body has received the grant money for. This form must be completed by the Auspice body and submitted by the applicant at the time of reporting.
